This additional form can be used when sending your 2008 Small Grains Sample to the OK Dept. of Agriculture Seed Lab for testing.

                                                                                      are the only forms that can be sent by email or fax.  Form C and

                                                                                      Transfer Report Form require signatures so they must be mailed.
